Requirements for PLDT Unli Fiber Plan Application:

Before proceeding with the application, please follow these steps:

  1. Step 1: Capture Photos of your ID
    To complete the PLDT Online application, you need to provide two photos of your ID: one of the front side and one of the back. Ensure that all the details on the ID are clearly readable in the photos, specially your name, photo and signature.

  2. Step 2: Take a Selfie Photo with your ID
    As part of the PLDT Online Application, you need to take a selfie while holding your valid ID(s). Make sure your face is clearly visible in the photo, without any accessories obstructing your facial features (e.g., sunglasses or mask). The identification card details must be readable in the picture.

  3. Step 3: Fill out the Online Application Form
    Click on the "APPLY NOW" button and complete the online application form with accurate and complete information. Many applicants tend to overlook certain details, which can cause delays in the processing of their application. Please ensure that you provide all the required information, including your house number, street name, barangay name, and two contact numbers. One contact number should be your own, while the other can be a contact number of a family member. To activate the "SUBMIT" button at the bottom of the application form, please fill in all the required fields.

    NOTE: Please provide your FULL MOTHER'S MAIDEN NAME name in the remarks section near the bottom of the application form.

    IMPORTANT: Please check only the "I have read and agree to the Subscriber Declaration above.*" and the "I have read and agree to the Subscription Contract.*" on the SUBSCRIBER DECLARATION check boxes. Checking the other boxes (eg: I consent... I allow..) will cause a DELAY in your application, so please take note of this. If you have a cignal inclusion in your plan, checking the "I have read and agree to the Cignal Privacy Policy*" is also required.

  4. Step 4: Submit and Confirm
    After entering all the necessary information click on the "SUBMIT" button. Review the information you have provided and confirm its accuracy.

A PLDT representative will contact you within 24 hours to confirm your application. Please ensure that your contact numbers are readily available during the application period.
